When a loved one is in need of end-of-life care, it can be overwhelming and difficult to navigate. This was the experience of Scott and his family when his kidney cancer returned.

His brother, Chip, knew that they were unable to care for him as needed due to the demands of work and family life. This is when they turned to SSM Health at Home Hospice Care.

The hospice team provided care beyond what Scott's family expected. SSM Health at Home initially provided palliative care under the Home Health program for symptom management.

When Scott transitioned to hospice care, they were relieved to have the continuity of care and attention from their experienced caregivers.

The hospice team took care of every need of Scott with compassion and attention to detail. The team ensured Scott was comfortable and helped relieve any pain or discomfort he may have had. Chip described his brother easing in and relaxing every time the nurses were around.

The hospice team provided emotional support to Scott throughout his final days, reassuring him that he was safe and that he was being cared for professionally.

The hospice team provided personalized support to Scott, making him comfortable in every way they could, even rearranging the blinds so that the sunlight would not be direct. The team's attention to detail gave Scott relief from his anxieties, which tended to creep up in the room.

The family's emotional support during this time was also taken into account by the hospice team, as they know the entire family is affected by this journey. The team eased the family's burden by regularly communicating with them and ensuring every family member was comfortable with the care assimilated.

Scott's final days were dignified and comfortable. After he passed, the hospice team supported the family by helping them cope with the loss and making the necessary arrangements according to Scott's wishes.

Chip expressed his gratitude towards the hospice team for the care they provided to his brother and the entire family.

SSM Health at Home Hospice Care provided the support that Scott's family needed in their time of need. The team's level of personalized care and compassion allowed Scott to be at peace.

With their supportive and empathetic care, the hospice team continued to stand by the family throughout Scott's journey to provide them with the utmost care and end-of-life comfort.
